There is an option in ACP to allow users to use delimiters in their usernames. The delimiters include "[" and "]" symbols.

The new quote functionality (implemented for 3.2: https://tracker.phpbb.com/browse/PHPBB3-10620) tries to parse the quoted post author. If this post author has a  [ or ] in their username, this results in a broken quote code.
