Extension classes should not break forum if not available

When an enabled extension class is not available, phpbb currently fails in multiple ways, and sometimes it is even quite difficult to recover from the situation without an expert administrator. There should be a way such that an extension class failing to load should automatically disable the corresponding extension, making the rest of the forum (and other extensions) continue working normally. This is not 100% achievable, but the closer to the objective, the better.
