Disallow usernames should not be case sensitive

Presently, if you disallow the username, for example, "phpbb", the username "phpBB" is not disallowed.  This seems wrong.

If you want to disallow, say, "motherf****er", you probably want to ban all case variations of the same.  The Disallow usernames ought to act on username_clean.
