Issues in PM report emails

When an admin receives an email about a reported PM I can see a few issues in it:

1) The subject says »Private Message report - ""« - there is no text between "".

2) The email says that »a Private Message titled "[subject-of-the-pm]" by "[name-of-the-user-who-reported-that-pm]" (...) was reported« - I believe that [name-of-the-user-who-sent-that-pm] should be used here. Or else the sentence should be rephrased, e.g. »You are receiving this notification because a Private Message titled "{SUBJECT}" at "{SITENAME}" was reported by "{AUTHOR_NAME}".«

3) The link provided in the email does not work - "http://forum.example.commcp.php/?r=" is used instead of "http://forum.example.com/mcp.php?r=" ("/" is at wrong place).
