Catch exceptions and display a nice error page

    Description

      A number of exceptions has not catched in phpBB, that resulting in PHP Fatal Errors, that are not displayed when DEBUG is disabled.

      I think that, we need to set an exception handler that display a general error, such as msg_handler (called by trigger_error).
