Remove all :link, :visited, :active link states

The use on :link is deprecated in all the browsers that we currently support leading to specificity problems.

Furthermore the use of active and visited are also leading to bloated codebase and a lot of duplication of effort. These pseudo rules are just one of the many things preventing us from moving towards a more modular code base.
