Separate vendor and package in namespace

Change namespaces to include a vendor and a package so:
phpbb/phpbb/extension/manager instead of phpbb/extension/manager

This is to follow best practices and avoid any conflicts in the future between phpbb core and any other libraries/packages we might release under the phpbb vendor namespace.
