Refactor Reports System

The current reports system has a few drawbacks:

Only posts/pms can be reported
Data from the posts/pms are copied to the reports table, duplicating data that should exist in full once post history is recorded (post revisions)

Some sort of system that supports plugins would be desirable so that mods could add new types or reports that could be displayed without editing code.
