Remove $load_extensions and weird dl() calls

It is possible to define $load_extensions in common.php. This string is split by comma, and each item is loaded via `dl()`. dl is broken and has already been removed from some SAPIs with PHP 5.3.0.

This feature is really useless, we should just scrap it.
