Add ext/index.html

At the moment as the ext/ directory doesn't exist until you install an extension. However installing an extension means the ext/ file doesn't have an index.html and means the folder will show the whole directories contents on some servers.

I suggest that it is removed from the .gitignore, an index.html is committed and it is re-added to the .gitignore.
