sql_create_table doesn't check correctly index length

Since phpBB 3.0.9, sql_create_unique_index and sql_create_index methods check length index to prevent issue with some Dbms (firebird and Postgresql) As we remarked with nickvergessen, this check seems to not be properly done when sql_create_table is called directly.
