      Bug found at http://www.phpbb.com/community/viewtopic.php?f=46&t=1861645&p=12801186#p12801186

      The CAPTCHA Module Settings module in the ACP says:

      "Enable visual confirmation for registrations:
      Requires new users to enter a random code matching an image to help prevent mass registrations."

      Not all of the CAPTCHA modules are a random code which is slightly confusing.
