      The ext/ dir should be created when composer dependencies are installed. This way it does not have to be done manually. This can be done like this http://getcomposer.org/doc/articles/scripts.md using the post-install-cmd event by running the mkdir command.

      We decided to just add ext/index.htm to the repository.
