Including a default Favicon for fresh instalations

      According to http://area51.phpbb.com/phpBB/viewtopic.php?f=3&t=41383 favicon.ico should be added to fresh installations of phpBB in root directory of phpBB.

      Favicon cannot be added to <head>!

      Also patch needs to include check that ensures favicon.ico is added just to fresh installations, not updated boards.
